PREOPERATIVE CARBOHYDRATE LOADING AND ANTI-INFLAMMATORY RESPONSE IN WOMEN UNDERGOING BREAST CANCER SURGERY

Rationale: Surgical trauma triggers a complex inflammatory response characterized by the release of pro and anti-inflammatory mediators. Interleukin-10 (IL-10) has anti-angiogenic properties and plays a key role in inflammation control and restoration of homeostasis. This study aimed to evaluate the effect of preoperative carbohydrate loading on the inflammatory response in breast cancer surgery.

Methods: A non-randomized clinical trial was conducted including 42 women undergoing elective breast cancer surgery. Participants were allocated to a control group (conventional fasting) or an intervention group receiving a 12.5% maltodextrin solution before surgery. Inflammatory markers were assessed at three times: preoperatively, at 4 and 12 hours postoperatively. Longitudinal analyses were performed using adjusted mixed-effects models.

Results: The intervention group underwent longer and more extensive surgical procedures, including higher rates of radical mastectomy and lymphadenectomy. Despite the greater surgical complexity, patients who received preoperative maltodextrin presented significantly higher IL-10 concentrations throughout the perioperative period.

Conclusion: Preoperative carbohydrate loading with maltodextrin demonstrated the potential to favorably modulate the inflammatory response to surgical trauma. The higher IL-10 concentrations observed in the intervention group suggest enhanced activation of anti-inflammatory mechanisms, even among patients undergoing more extensive surgical procedures. These findings support the role of perioperative nutritional strategies in modulating inflammation during breast cancer surgery.
