DIAGNOSTIC VALUE OF OBESITY ASSESSMENT CRITERIA IN OVERWEIGHT AND OBESE WOMEN IN EARLY PREGNANCY

Rationale: Body mass index (BMI) is the common indicator of obesity evaluation criteria before pregnancy, however, there are no diagnostic criteria for other commonly used obesity indicators in early pregnancy, such as waist-to-hip ratio (WHR) and visceral fat area (VFA). This study aimed to explore the diagnostic value of obesity, in order to find more specific obesity evaluation criteria and provide basis for health guidance in overweight and obese pregnant women in the first trimester.

Methods: Totally 286 overweight and obese women were enrolled in early pregnancy in Beijing between June 2023 and March 2024, and WHR and VFA were measured by body composition analyzer. The pregnant women were divided into two groups: overweight group (24-27.9 kg/m2) and obese group (> 28kg/m2) according to Chinese BMI classification criteria. Spearman's correlation was conducted to examine the associations between VFA/WHR and BMI. The optimal cut-off values for diagnosing obesity by using WHR and VFA was explored through the receiver operating characteristic (ROC) curve.

Results: There were statistically significant differences in VFA and WHR between overweight and obese groups (P<0.05). VFA was positively correlated with BMI (r=0.722, P<0.01), WHR (r=0.650, P<0.01), BMI was positively correlated with WHR (r=0.419, P<0.01). The cutoff value of VFA for obesity prediction was 144 cm², and the area under the curve (AUC) was 0.892 (95%CI: 0.854~0.932). The cutoff value of WHR for obesity was 0.93 and the AUC was 0.727 (95% CI: 0.664 to 0.786).

Conclusion: VFA showed strong positive correlations with BMI (r>0.7) among overweight and obese group. ROC curve analysis showed that VFA had high diagnostic accuracy for obesity (AUC=0.892), and the cutoff value of 144 cm² could be used as a reference for evaluation, while WHR had moderate diagnostic efficacy (AUC=0.727), and the cutoff value of 0.93 could assist judgment.
