Rationale: The introduction of CFTR modulators has revolutionized the treatment of CF, shifting clinical attention to the metabolic implications of weight regain.

Methods: Retrospective observational study conducted on 223 patients (Adult section, Piedmont and Valle d'Aosta), focusing on 102 receiving CFTR and comparing pre-therapy and 4-year results.

Results: The treated cohort (n=102) showed a statistically significant increase in lung function, weight and BMI. Metabolic investigation revealed elevations in total cholesterol and HDL, correlated with weight recovery. Stratification by pancreatic function showed: Pancreatic Sufficient (PS) with higher baseline profile and marked post-therapy increase, the prevalence of hypercholesterolemia (>200 mg/dL) increased from 30% to 43%; Pancreatic insufficiency (PI): lower mean values, with hypercholesterolemia increasing from 3% to 14%.

Conclusion: Modulators improve respiratory and nutritional outcomes, but lipid changes need a team approach. Dietitians play a key role through personalized nutrition counseling to reduce metabolic issues and avoid harmful drug interactions. At our Center, dietetic intervention has helped maintain healthy weight gain, improve HDL cholesterol, and optimize lifestyles. A focus on a healthy diet is essential to prevent obesity and related health problems.
