MALNUTRITION RISK INCREASES WITH DISEASE PROGRESSION IN ACUTE PANCREATITIS: A PROSPECTIVE COHORT STUDY USING MUST AND GLIM CRITERIA

Rationale: Acute pancreatitis causes metabolic stress and catabolism, leading to rapid nutritional decline. This study assessed patients’ nutritional status and its association with malnutrition and disease progression.
Methods: A prospective cohort of 3,014 patients at Semmelweis University was analyzed. Nutritional status was assessed using BMI, MUST, and GLIM, and logistic regression examined the association between pancreatitis episodes and malnutrition, using first-episode acute pancreatitis as the reference.
Results: Overall, 25.3% of patients were classified as being at risk of malnutrition according to MUST, while 21.2% met the diagnostic criteria for malnutrition based on GLIM. Disease progression showed a significant association with both malnutrition risk and diagnosed malnutrition. Compared with first-episode acute pancreatitis, the odds of MUST-defined malnutrition risk were higher in recurrent acute pancreatitis (OR=1.32; p=0.029), early chronic pancreatitis (OR=1.55; p=0.003), and acute-on-chronic pancreatitis (OR=3.06; p<0.001). A similar pattern was observed for GLIM-defined malnutrition: recurrent acute pancreatitis (OR=1.36; p=0.018), early chronic pancreatitis (OR=1.61; p=0.001), and acute-on-chronic pancreatitis (OR=3.32; p<0.001). BMI-defined undernutrition did not show a significant association with pancreatitis stage.
Conclusion: Malnutrition increases with disease progression: fewer patients are affected at the first episode, while nearly half of chronic cases are at nutritional risk, highlighting the need for increased nutritional management as the disease advances. MUST showed similar sensitivity to GLIM.
