Rationale: Few studies investigated the association between endogenous HMB levels and muscle health, and how longitudinal changes in HMB levels correlate with muscle health outcomes after intervention with oral nutritional supplement (ONS) with HMB.

Methods: This randomized, double-blind, placebo-controlled trial consisted of 811 community-dwelling older adults (≥ 65 years old) at risk of malnutrition. In the intervention group, participants were given an ONS with HMB (0.74 g calcium HMB per serving) and a placebo was given to the placebo group. Both groups received dietary counselling and consumed study products twice a day for 180 days. Plasma and urinary HMB levels, muscle mass, handgrip strength (HGS), and Short Physical Performance Battery (SPPB) were measured at baseline, day 90, and day 180.

Results: A total of 604 participants provided fasting blood samples and/or urinary samples for HMB analyses. At baseline, participants with high vs. low fasting plasma HMB levels (median split 175 ng/mL) showed significantly higher muscle mass, ASMI and HGS, and lower fat mass (all P ≤ 0.019). Participants with high urinary HMB levels (median split 3699 ng/mL) had significantly larger calf circumference, higher muscle mass, HGS, and SPPB score (all P ≤ 0.043). ONS with HMB significantly increased plasma and urinary HMB levels at days 90 and 180 (all P < 0.001), with positive correlations between the change in HMB levels and ASMI, SPPB, and 5-time chair stand test scores at day 180 (all P ≤ 0.036).

Conclusion: In community-dwelling older adults at risk of malnutrition, higher endogenous HMB levels were associated with better muscle health. Consuming ONS with HMB for six months significantly increased HMB levels, and these increases were associated with improvements in muscle mass and physical performance.
