Rationale: Metabolic dysfunction–associated steatotic liver disease (MASLD) is getting more prevalent in older adults. This study aims to identify MASLD predictors and evaluate their link to nutritional and functional reserves in the older adults.
Methods: A retrospective study conducted at a geriatric outpatient clinic in a tertiary care hospital. MASLD was identified using the Hepatic Steatosis Index (HSI) and cardiometabolic risk factors. Demographic characteristics, comorbidities, medication use, anthropometric measures, and geriatric assessment tools were evaluated. A logistic regression analysis was performed to identify factors independently associated with MASLD.
Results: A total of 1395 patients (63.6% female) were analyzed with a mean±SD age of 74.16 ± 6.40 years. MASLD was present in 57.3% of participants. According to the MNA-SF, 6.0% of patients were malnourished and 21.6% were at risk of malnutrition. The rate of polypharmacy and hyper-polypharmacy was found to be 65.2% and 17.2%, respectively. The multivariable logistic regression analysis found hip circumference (OR=1.189, CI:1.150-1.229, p<0.001), hypertension (OR=1.720, CI:1.073-2.756, p=0.024), serum gamma glutamyl transferase (GGT) enzyme level (OR=1.018, CI1.009-1.028, p<0.001) and very low density lipoprotein (VLDL) levels (OR=1.018, CI:1.000-1.035, p=0.045) and hyper-polypharmacy (OR=1.896, CI:0.977-3.677, p=0.058) as independent positive predictors for MASLD.
Conclusion: MASLD is common among older patients. The risk of MASLD may be assessed in older patients with hypertension, large hip circumference and the presence of hyper-polypharmacy. Additionally, the follow-up of serum GGT and VLDL levels may be beneficial for MASLD risk assessment.
