Rationale: Diagnosing sarcopenia in Type 1 Diabetes (T1D) is challenging due to phenotypic overlaps like hidden adiposity. We evaluated whether different bioelectrical impedance analysis (BIA) equations accurately reflect true cellular integrity, measured by phase angle (PA).

Methods: This cross-sectional study included 194 adults with T1D (mean age 55.7 years, disease duration 27.6 years). We performed BIA (Akern, 50 kHz) and physical function tests. We compared sarcopenia prevalence using Janssen and Sergi equations. Adiposity was defined using Kyle criteria. Multivariate linear regression adjusted for age and sex evaluated the independent association of each sarcopenia definition with PA.

Results: Hidden adiposity increased with age, peaking in women >65 years (17.6%). Sarcopenia prevalence varied significantly by equation: Janssen (28.4%) versus Sergi (8.7%). In multivariate models adjusted for age and sex, sarcopenia diagnosed by the Sergi equation strongly predicted a severe decline in PA (p=0.003). The Janssen equation did not independently predict cellular risk or muscle strength after sex adjustment. Furthermore, patients with PA-defined cellular risk exhibited significantly lower handgrip strength (27.7 vs. 34.4 kg, p=0.005).

Conclusion: Conclusions: Standard body mass index masks substantial adiposity in older adults with T1D. For diagnosing sarcopenia via BIA, the Sergi equation is superior to the Janssen equation in T1D, as it independently correlates with cellular membrane damage (PA) and functional decline. PA serves as a precise, early biomarker for structural muscle quality in routine clinical practice.
