Rationale: Pulmonary hypertension (PH) causes cardiac and pulmonary deterioration.(1)Patients had reduced lean mass, respiratory strength and endurance, increased body fat, a higher cardiac complications and risk of mortality.The prevalence of body composition alterations remains uncertain.(2)Our aim was determining the prevalence of body composition abnormalities in patients at moderate-to-high risk of PH.
Methods: A cross-sectional study.A total of 252 patients >18 years with moderate-to-high risk of PH(maximum tricuspid regurgitation velocity >2.9) were included.Patients hospitalized in the < 3 months were excluded.Body composition was assessed by bioimpedance.Dynapenia,muscle wasting,and sarcopenia were defined according to EWGSOP2 guidelines,and pulmonary function was evaluated by spirometry.The comparison between the study groups was performed using x2 for qualitative variables and t-student for quantitative variables.
Results: A total of 252 patients were evaluated,47.61% were women.The mean age was 66.85±12.71, 50% had systemic hypertension,38% COPD,41% ILD, 84% HF,79% abdominal obesity,33% muscle wasting,and 45% dynapenia.Women showed a higher prevalence of hypertension (57% vs 44%, p=0.042),lower nephropathy(1% vs 14%, p<0.001), FEV1(1.22±0.43 vs 1.58±0.66, p=0.002),and FVC(1.85±0.55 vs 2.53±0.69, p<0.001)compared to men. Regarding body composition, women had a higher prevalence of sarcopenia(16% vs 5%, p=0.007) and abdominal obesity (90% vs 70%, p<0.001),and lower muscle wasting(25% vs 41%, p=0.013)compared to men.

Conclusion: In patients with moderate-to-high risk of PH, sarcopenia and abdominal obesity are more prevalent in women and may be associated with a worse prognosis.
