Rationale: Sarcopenia has significant impact on aging populations. However, combined predictive value of simple body measurements, physical strength, and metabolic markers is still unclear.
Methods: This prospective cohort study, 4 years follow up included 3,469 middle-aged and older adults. Sarcopenia was defined using AWGS 2019 criteria. Baseline assessed calf circumference, handgrip strength, waist circumference,Triglyceride-Glucose (TyG) index. Cox proportional hazards models evaluated independent and joint associations while adjusting for confounding factors, conducted interaction, sensitivity analyses.
Results: Higher calf circumference and handgrip strength linked to lower risk of sarcopenia. Waist circumference and TyG index also showed negative associations. Favourable combined profiles demonstrated strongest protective effect (HR: 0.25; 95% CI: 0.16 to 0.38), with evidence of synergistic interaction.

Conclusion: Evaluating calf circumference, handgrip strength, waist circumference, and TyG index together improves early identification of sarcopenia risk. This offers practical and economically efficient strategy for clinical practice. These exploratory observations highlight complex relationship between metabolic profiles, body composition, and musculoskeletal health in older adults. Using these indices together can enhance accuracy of identifying individuals at higher risk and guide targeted prevention and intervention strategies.
