SHIFT WORK IS INFLUENCING THE DEVELOPMENT OF METABOLIC DYSFUNCTION ASSOCIATED STEATOTIC LIVER DISEASE COVERTLY

Rationale: Shift work has been implicated as a risk factor for metabolic dysfunction associated steatotic liver disease (MASLD). Robust data on the risk of MASLD in this population are lacking.
Methods: This cross-sectional study was conducted among 48,374 participants included in the Kailuan cohort study between 2016 and 2018. The primary exposures were shift work and sleep. Participants completed a questionnaire, reporting having worked a night shift as a shift worker, and scored their sleep using the Pittsburgh Sleep Quality Index (PSQI), with a PSQI score of ≥5 indicating poor sleep quality. Multivariable logistic regression was used to calculate adjusted odds ratios and 95% confidence intervals for the risk of MASLD.
Results: Among the 48,374 participants, 41.2% (20,018/48,374) had MASLD. Of the respondents, 32.4% (8,093/48,374) were shift workers, and 38.5% (7,049/48,374) were rated as poor sleepers. MASLD was positively correlated with shift work (OR: 1.18, 95% CI: 1.11-1.24) and poor sleep quality (OR: 1.84, 95% CI: 1.74-1.86), respectively. Compared with the group without shift work and good sleep, the group with shift work and poor sleep had a higher risk of MASLD (OR: 2.55, 95% CI: 2.26-2.88).
Conclusion: A high risk of MASLD was associated with shift work and poor sleep quality. The high incidence of MASLD among mining workers highlights the underestimation of outcomes associated with this condition. We suggest avoiding long-term night shift work and maintaining good sleep quality to reduce the risk of MASLD.
