Rationale: Sarcopenia and malnutrition are common in patients with advanced chronic liver disease (ACLD) and are associated with adverse clinical outcomes. Simple bed-side tools are needed for early identification of both physical statues. This study aimed to find the association between, sarcopenia risk screened with SARC-F, and nutritional statues assessed by Subjective Global Assessment (SGA).
Methods: A cross-sectional analytical study conducted in the two major liver transplant centers of Sri Lanka among adult patients with ACLD awaiting liver transplantation. Nutritional status was assessed by SGA while sarcopenia risk was screened by SARC-F questionnaire. Participants were selected using simple random sampling. The association between nutrition status and screening score of sarcopenia were analysed using Chi-square tests and Spearman correlation.
Results: A total number of 190 were included. The prevalence of malnutrition based on SGA was 90.53%, while oedema free BMI was within normal range in 51.6% of patients. High risk for sarcopenia (score ≥4) was identified in 45%. The median duration of ACLD was 3.6 years. A positive correction was observed between SARC-F scores and SGA category (Spearman’s p ~ 0.7). Patients with high SARC-F scores demonstrated moderate to severe functional impairment significantly corresponded to more severe malnutrition (x2 ~ 40, p < 0.001).
Conclusion: SARC-F shows a strong association with nutritional status assessed by SGA in patients with ACLD. It may serve as a simple and a practical screening tool to identify patients at high risk of sarcopenia and malnutrition, particularly in resource-limited settings.
